Camp America and Summer Camp Forms

The GP Partners have advised that the practice will no longer be completing the Summer Camp medical reports like we have done in previous years. This is non-NHS work and requires time outside of regular GP clinics to complete. Patients can request copies of their medical record instead, or go to a Private GP to get the medical form completed.
