Adult ADHD Referral Process
Thank you for your interest in a referral for an Adult ADHD assessment. This document outlines the available referral options and the steps you need to follow. Please read carefully before proceeding.
Nottingham Adult ADHD Pathway (NHS)
Approximate waiting time: 2 years
Before requesting referral to the Nottingham ADHD pathway, please ensure that you will remain living in Nottingham throughout the assessment and treatment period. If you relocate during this time, your referral will be cancelled, and you will need to restart the process in your new area.
Steps to start the process:
1. Download and complete the ADHD referral form. – Provide as much detail as possible, including your symptoms and examples from childhood. – Referrals with insufficient information will be rejected.
2. Complete the three accompanying questionnaires.
3. Submit your request. – Send an ADMIN request with the completed form and questionnaires attached. – Confirm in your message that you would like to be referred to the Nottingham ADHD pathway. – You will receive a text message once your referral has been processed and sent.
If your referral is rejected by the Nottingham ADHD team, you will be notified by text with the reason. It will then be your responsibility to either provide additional information or book an appointment with your GP to discuss next steps.
Important:
– If we request additional information, we will hold your referral for up to 21 days.
– If you do not respond within this timeframe, we will assume you no longer wish to proceed, and your request will be closed. This helps ensure our admin team can support other patients promptly.
Right to Choose Pathway
Due to the long waiting times for the Nottingham NHS pathway, you may wish to use the Right to Choose (RTC) pathway. This allows you to select an NHS-approved private provider, with costs covered by the NHS.
Please note:
– All RTC providers are private organisations.
– You are responsible for choosing your provider—our practice cannot do this for you.
When selecting a provider, ensure that:
· You meet their referral criteria.
· They meet the NHS shared care criteria. If they do not, we cannot provide ongoing prescribing or monitoring. We strongly recommend contacting the provider in advance and confirming this before proceeding by downloading the agreement and sending it to them.
· They are able to prescribe medication (if required), and are not solely a diagnostic service.
· You understand their waiting times and services offered.
⚠️ Do not select a provider based solely on waiting times—please ensure the service is suitable for your needs.
Steps to proceed via the Right to Choose pathway:
1. Complete the ADHD screening questionnaire.
2. Provide details of your chosen RTC provider.
3. Submit an ADMIN request including: – The ADHD screening questionnaire. – Your selected provider. – A detailed description of your symptoms, concerns, and the impact on your daily life.
You will then receive a text message confirming either:
– That your referral has been sent to your chosen provider, or
– That we require further information before processing your request.
Once your referral has been sent, the GP surgery will no longer be involved in your care. All queries must be directed to your chosen provider.
