Disabled Students’ Allowance (DSA) Forms

1. I’m applying for a DSA, how do I get medical evidence?

You will likely need to provide medical evidence of your long term health condition to your university in order to apply for a DSA. Student Finance England (SFE) have created a form for you to take to the GP or other medical professional. You can get the DSA form from the NTU website at the bottom of this page.

2. I have the DSA form, how do I give it to my GP?

Please submit the form to us online via a Patient Triage Request or hand it in to the surgery. Make sure your personal details are completed on the front page. Once you have submitted the form to us, it will be passed onto the relevant member of our team.

3. How long does it take for the GP to complete?

We action DSA Medical Evidence forms within 21 days of receipt. There is no charge for these. We cannot complete these forms urgently. We will contact you once the form has been completed.
