If you’ve been off sick for more than 7 days
To request a sick note:
- fill out a sick note request form
- phone the surgery when we are open.
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

University medical notes
GP Medical Note or Self Certification?
If you are unwell for less than 7 days and have not been able to attend exams or assessments, you can submit an NTU Self Certification form to your university here: NTU Self Certification Form
We will only provide a GP medical note (if appropriate) for an illness over 7 days & if you have been seen by medical/nursing staff whilst being unwell. We CANNOT do medical notes if you have not been seen by a GP/Clinician at the surgery whilst you are unwell.

GP Medical Notes (NEC’s) – How to apply
There is a minimum fee of £30 for this service, and letters will be completed within 21 days from payment (non-negotiable).
- Fill in the form at the bottom of this page and email it to nnicb-nn.c84117@nhs.net
- Wait for a member of the team to contact you about your request
- If a Medical Note is appropriate, we will ask the GP if they are able to do a letter
- We will then contact you to arrange payment
- Once payment has been made, the letter will be typed and signed by a GP within 14 days
- The letter will be emailed to you or you will be contacted to collect it
Why is there a charge for this?
The charges for such services are based on the time taken to do a review of your health record, current and past, and the administrative time in preparing the medical note. Do not leave requesting an NEC until the last minute as we may not be able to complete your request in time. We are not compelled to complete any non-NHS work, but do so for the convenience of our patients but never to the detriment of our NHS duties, which always comes first.
What about DSA’s or Access Statements?
DSA and Access Statement forms DO NOT fall under this category and are completed free of charge, if you are eligible. These forms will be completed within 21 days (non-negotiable). Read more about the Disabled Student Allowance and Access Statements
Other places you can get Medical Evidence:
- The NHS App
- University self-certification form
- Hospital/clinic letter provided by the hospital
- Letter provided by your counsellor/psychologist
- Crime report and number
- Documentary evidence from a court or solicitor
The GP will not provide a medical note for the following conditions/circumstances:
- Failure to read the exam timetable/assessment deadlines
- Exam stress/pressure
- Minor illnesses (e.g., coughs and colds etc.)
- Self-induced conditions (e.g., hangover)
- Personal disruptions (e.g., holidays)
- Accommodation/housing disturbances
